How to book?
For any patients looking for an appointment, please give us a call, email, text and/or fill out the form provided on the website and a member of our team will reach out to you as soon as possible. For emergencies please call our office immediately as that is the quickest way to reach us.
What do I have to fill out?
New patient forms are sent via email. Forms should be filled out prior to your first visit as it contains important for pre-treatment information. For example allergies to an antibiotic needs to be disclosed in the medical history form to make sure that you do not get the wrong antibiotic. If you did not receive the forms please give us a call and we can resend the forms. Patients should try their best to fill out all forms, unless having technical difficulties or have a hard time understanding the forms.
Insurance Coverage Questions?
For any insurance coverage confusions, our staff can assist you through it. The office offers direct billing for all insurances which saves you the hassle of having to submit claims yourself. For student coverage information all Ontario college and university insurances are listed below the student insurance tab.
What Should I bring to my first appointment?
On the day of appointment, please bring a piece of ID or a picture of that ID. If you are insured bring your insurance card, a picture of the card or other insurance information as those will be necessary for us to direct bill to the insurance companies. If you have visited another dental office before, please ask for a copy of your dental x-rays or have the contact information of the office ready so that we can send a dental record request form.
